How can we help you today?

Non NHS forms and reports (private work)

Due to operational necessity, we have made the decision not to undertake any non NHS work; our priority must be to ensure we can deliver NHS services we are contracted to provide to our patients. Private work is outside the remit of the NHS contract and as such, practices are not obliged to provide this.

Please see below as examples of some of the work we are not contracted to do :

  • Medicals and forms for employment, sports and driving requirements (HGV, PSV etc)

  • Medical reports and examinations for insurance companies and employers

  • Insurance claim forms

  • Passport signing

  • Private prescriptions

  • Private sick notes

  • Vaccination certificates

  • Holiday cancellation forms

  • To whom it may concern letters

  • Forms, certificates and medicals

  • Fit to fly letters/certificates

We respectfully request that you source your own private GP to provide you with this service

Have you ever thought about joining our Patient Participation Group (PPG)?

Farrier House Surgery are looking for volunteers to join our PPG. We aim to have a group from as broad a spectrum as possible and meetings will be arranged to make the group inclusive for all.

We need young people, students, workers, unemployed, carers, retirees, people with long-term conditions or disabilities and people from non-British Ethnic groups

The group will comprise of;

• Chair Person

• Secretary

• Treasurer (if fundraising is involved)

• Carer (s)

• Group members (patients from across the surgery representing the practice population)

• Clinical and non-clinical staff

What are PPGs?

PPGs are a group of patients and members of staff based out of a GP practice. They work to improve the overall experience of the patient population registered at that practice.

Who sits on a PPG?

PPGs are usually made up of a group of volunteer patients and members of staff based at the Practice.

What do PPGs do?

PPGs should meet on a regular basis to discuss the services on offer. They will look at how improvements can be made for the benefit of patients and the practice. Depending on the skills, availability, and motivation of those involved, what you can do in your PPG can vary.

What the PPG is not about

The group will not deal with personal medical issues or individual patient complaints as there is already an existing procedure to handle such matters.

If you would like to join our PPG, please click on Have Your Say at the top of this page and register your interest. You will also have the option of declaring an interest in specific roles. Diversity of the group will be the only factor in confirming selection, if we receive a lot of interest.
